The field of assisted reproductive technology (ART) continues to gain recognition for the benefits it provides to patients that have unexplained infertility, repeated pregnancy loss, or a need for genetic and X-chromosome determination. Results reported at the most recent meeting of the American Society for Reproductive Medicine (ASRM) indicate that PGD-screened patients have higher implantation rates, higher pregnancy rates, and lower pregnancy loss rates when compared to an age-matched group of non-PGD-screened patients. The implications are that women with challenges such as advanced maternal age (>35 years), unexplained infertility, repeated pregnancy loss, and a need for genetic and X-chromosome determination can take advantage of the new opportunities available. It is evident that PGD has achieved a significant increase in acceptance and has become a new way for positive fertility results to be achieved.

The history and science of PGD are both fascinating and powerful. PGD was first used in England in 1989, but the process has been refined greatly since its early stages. It involves the removal/ biopsy of a single cell from early cleavage stage embryos. The nuclear material from that cell is evaluated via fluorescent in-situ hybridization (FISH) or polymerase chain reaction (PCR). PGD is used to evaluate the genetic makeup for chromosomal abnormalities (e.g., Down’s Syndrome, translocations, X-linked disorders) or screen for the presence of genetic disorders (e.g., Cystic Fibrosis, Tay-Sachs).
